Subject: [PATCH] fix(ai): allow hex-like strings in security review issue

- Add issues/** and scripts/agent/** to check-hex.js ignore list
- This prevents CI lint failures when documentation or scripts contain hex-like strings (e.g. in URLs or comments) that are not CSS colors.
