fix: exclude examples from build and restore permissions

- Excludes `examples` directory from `dist/` in `scripts/build-dist.mjs` to prevent reproducer code from being deployed.
- Restores `DEFAULT_NIP07_PERMISSION_METHODS` in `js/nostr/client.js` login flow to fix unit test failures in `nostr-login-permissions.test.mjs`.
- Adds a robust reproducer for the Moderation UI bug in `examples/reproducers/issue-moderation-ui/`.
